To submit a damage claim, please completely fill and submit the form below. You must attach pictures of the damaged item(s), plus one of the following:
- Probable cause of damage from an authorized service center.
- Written estimate or actual receipt for the repair cost from an authorized service center.
- If the unit is not repairable, a receipt for actual or estimated replacement cost from a retailer.
The information you provide will be combined with our log of the incident and sent to our insurance company. They will investigate the claim and render a decision, usually within 2 weeks. Note: Retain the damaged items for inspection until a decision has been rendered.
Electric service is intended to be continuously available. It is inherent, however, that there will at times be some degree of interruption or fluctuation. While we strive to provide this reliability, the cooperative does not guaranty constant or uninterrupted delivery of service. This will occur in the normal operation of the electric system (changing equipment, shifting load, etc.).
The member is responsible to provide their own insurance or devices adequate to protect his/her equipment from damage that may be caused by voltage fluctuations, single-phasing, long term outages or other events that are out of the cooperative’s control.
Things that are out of the cooperatives control include but are not limited to “acts of nature” (lightning, snow, ice, wind, etc.), contacts made by birds or animals, auto accidents, or mechanical failures.
